This is a wonderful questions. There are many different ways to help a horse struggling to gain fat and/or muscle but here is what I would recommend:
-Feed your horse 3x per day, with alfalfa being 25-50%
-Feed your horse 2-5 lbs of soaked beet pulp (high fiber and palatable) with 2-5lbs of rice bran (high in fat) and add flaxseed pellets or oil to help with omega3 omega6 intake; you can also add 1 TBSP of salt to help with your horse increasing water intake and getting enough electrolytes
P.S. if your horse doesn't like beet pulp and you don't have access to alfalfa hay, buy alfalfa cubes or pellets and soak about 3lbs worth and add 3-5 lbs rice bran. If you're worried about your horse's stomach being upset from all this grain, buy aloe juice and add it to their grain to help soothe their stomach acid and prevent ulcers. It is unlikely that a horse will develop anything from intaking more food when it is needed, but it is good to keep an eye out. Of course the more you put into their body the more excrement they will have! Your horse should be pooping a lot and gaining weight. I would use this regime for about 1-2 months and slowly decrease it over time as you see fit to their body condition.
